The problem can occur, tending towards bureaucracy, when no matter what the topic, there is a structure that as you put it "stifles" voices and creativity. Hierarchies can, indeed, take on characteristics of permancy when people get entrenched and then "calcify" in place. But anytime people are involved, the potential for change exists IF the leaders are diligent and encouraging: diligent about removing those who've calcified or encouraging them to change.
