I'm a developer based in New Zealand, and have been working remotely for a US based company for a few years now (Embarcadero Technologies, www.embarcadero.com). I would consider Skype to be our most valuable tool without a doubt. We have an ongoing team chat window always open, and make regular voice calls to combat the ASCII fatigue you mention.
I don't know if you necessarily have to be the type of programmer that bleeds 1s and 0s to make this work, but you definitely need to be self-motivated, and know when to ask your team for help if you hit a stumbling block.
