Love your use of cognitive biases and their causes. I've also witnessed this as a result of the combination of optimism bias and the sunk-cost fallacy when a particular tool or method has been implemented. Decision makers don't realize it's not working because they are blinded by their bias and resolve the cognitive dissonance whenever they get a sniff of the problems by some rationalization that seeks to discredit the messenger. That's why I love your approach of using a tool like Kanban as a discovery mechanism to enable kaizen.
