How about Talkatone? It promises free incoming and outgoing calls. I'm trying it on a jaibroken 3g 8mb (really a 2g) running ios 4.1. It shows promise although I have seen some erratic performaance, perhaps due to my ipt 2g. It uses Gmail Chat phone to call real numbers, and when set up with GV can receive calls. .
