Field API documentation (https://bim360field.autodesk.com/apidoc/index.html) would give a good overview of the capabilities or extent of what can be done with APIs.
When you say customized form are you referring to a Field Checklist? Then yes, you can create a checklist through API and also link additional data to it. Refer to api/checklist call.

Glue API Web and JS - Follow up
