Greetings - love the blog - Just as an aside Wikipedia documents 4 failures of the Titan IV platform. It goes to show that even when Failure is not a design option, it may be a implementation reality. This fuels the Agile principle of learn, build, learn within the context of the risk for what is being done. Thanks again for a fun and informative site.
